Rule Violation Reporting Form

Any individual may report alleged misconduct to the Federation, either verbally or in writing. Reports can be made anonymously to the Federation. There is no fee for filing this report. Any questions should be directed to the Regulation Department Director at makeareport@usef.org.

This Report Form is for any misconduct other than allegations under the USEF Safe Sport and MAAP Policies. To report allegations regarding the USEF Safe Sport or MAAP Policies, please utilize the USEF Safe Sport Incident Report Form.

Reporting Party

Please note that a member of the Regulation Department will need to contact you regarding this report. However, contact by a staff member will not jeopardize your request to remain anonymous.

Person Being Reported

Alleged Violation Information
If occurred at a competition, then where on competition grounds. If occurred outside competition grounds, address and description of where the incident occurred.

Witnesses
Provide the names and contact information for any other individuals who may have additional information regarding the alleged violation. To add more than one witness, select the Additional Witness checkbox and more witnesses can be added on a separate page.
