Rule Violation Reporting

Any individual may report alleged misconduct to the Federation, either verbally or in writing. Reports can be made anonymously to the Federation. There is no fee for filing this reports. Any questions should be directed to the Director, Regulation Department at [email protected].

This Report Form is for any misconduct other than allegations under the USEF Safe Sport and MAAPP Policies. To report allegations regarding the USEF Safe Sport or MAAPP Policy please utilize this Incident Report Form.

Please list Respondent's name and complete contact information to the extent it is known.

Please provide the following information if the incident occurred in connection with a USEF Licensed Competition.

Alleged Violation Information
If occurred at a competition, then where on competition grounds. If occurred outside competition grounds, address and description of where the incident occurred.
If yes, who did you report it to an when?
